Premenstrual syndrome (PMS) affects up to 75% of people who menstruate, with symptoms appearing in the luteal phase — the 1–2 weeks before menstruation — and resolving within a few days of the period starting. Symptoms span physical and emotional domains: cramping, bloating, breast tenderness, and headaches on the physical side; mood swings, irritability, anxiety, fatigue, sleep disruption, and food cravings on the emotional and behavioral side. PMS exists on a spectrum. Mild symptoms are a common part of the cycle for many people. Moderate symptoms can affect daily functioning. Severe symptoms — particularly when they significantly impair work, relationships, or quality of life — may meet the criteria for Premenstrual Dysphoric Disorder (PMDD), a more serious condition recognized in the DSM-5 that responds well to treatment including SSRIs, hormonal therapies, and lifestyle interventions. This calculator scores 10 common PMS symptoms on a 1–5 scale (1 = not present, 5 = severe) and returns a total score with a severity classification. How It Works
Each of the 10 symptoms is rated on a 1–5 scale (1 = not present, 5 = severely present), giving a minimum score of 10 and a maximum of 50. Scoring bands: Mild = 10–20, Moderate = 21–35, Severe = 36–50. The severe band (36+) aligns with the symptom intensity level that may meet criteria for PMDD, which is diagnosed clinically when at least 5 of 11 DSM-5 criteria symptoms are present in the luteal phase, cause significant distress or functional impairment, and are confirmed over at least 2 prospective cycles.
